APPLICANT NOTE:  This application form is intended for use in evaluating your qualifications for employment.  This is not an employment contract.  Please answer all appropriate questions completely and accurately.  False or misleading statements during the interview and on this form are grounds for terminating the application process or, if discovered after employment, terminating employment.  All qualified applicants will receive consideration without discrimination because of sex, marital status, race, age, creed, national origin or the presence of disabilities.  A felony conviction will not necessarily bar an applicant from employment.  Additional testing on job-related skills may be required prior to employment.  This application will remain active for four months.

1.  I certify that I have read and understand the applicant note above and that the answers given by me are complete and true to the best of my knowledge and belief.  I understand that any false information or any misrepresentation or omission of facts may result in rejection of my application or discharge at any time during my employment.

2.  I authorize the company and/or its agents, including consumer reporting bureaus, to inquire of and receive information on any or all statements contained in this application and also authorize any person, school, current employer (except as previously noted), and past employers named in this application to provide relevant information and opinions that may be useful in making a hiring decision.  I release such persons and organizations from any legal liability for any damage whatsoever for issuing this information.

3.  I understand that Vitamin Cottage may conduct a background check as part of the application process and I consent to such an investigation.

5.  I agree, if employed, to conform to the guidelines and regulations of Vitamin Cottage, whenever adopted by Vitamin Cottage, and that those guidelines and regulations do not constitute an employment contract and my employment and compensation is "at will."  I also understand that either Vitamin Cottage or I may terminate the employment relationship at any time, without notice, with or without reason.  Only a written agreement signed by the owners of this company may modify this paragraph.  

6.  This application is current for 4 months.  At the conclusion of this time, if I have not heard from Vitamin Cottage and still wish to be considered for employment, it will be necessary to fill out a new application.

7.  Please Note: Applicant agrees to provide the following:
     A. Proof of meeting minimum age requirements of applicable laws and submitting proof of true age after hired.   
     B. Submit proof of citizenship of U.S. and proof of employability for the immigration and Naturalization Service  (e.g. passport, drivers license, Colorado I.D. card and/or social security card).

8.  I understand that Vitamin Cottage is a non-smoking work place and do not allow smoking on the premises.
